Making Your Dental Appointments Stress-Free & Relaxing

At Rowley Family Dentistry, we believe that taking care of your pearly whites shouldn’t feel like a chore. That’s why we make sure all of our patients have everything they need to have a stress-free first visit with us. Below, you can find our new patient forms as well as some of the dental insurance companies that we’re in-network with. If you have any additional questions, please don’t hesitate to call us and ask!

Your First Dental Visit

We’ll collect your patient information and any specific questions or concerns that you have over the phone when you schedule your appointment to ensure that those are addressed when you come in. Once you arrive, we’ll collect any insurance information and your new patient forms and guide you back to one of our comfortable operatories. Here, you’ll get to meet with a skilled dental hygienist and Dr. Rowley. After examining your mouth and X-rays and making you aware of any findings, we’ll thoroughly clean your teeth before polishing and flossing them. Before you leave, we’ll schedule your next appointment or any follow-up visits with you.

We request that all of our new patients please fill out our forms, which you can download by clicking on the link below. That way, you don’t have to worry about filling them out in our waiting area. If you have difficulty accessing the forms, please call our office.
